Post-Discharge Follow-Up Calls in Community Hospitals

October 29, 2026

This is general information about practice operations, not medical, legal or compliance advice. Check the rules that apply to your organization.

The problem

After a hospital stay, patients go home with new medications, instructions and follow-up appointments. Confusion is common. Many hospitals make follow-up calls in the days after discharge to check understanding, confirm appointments and identify problems. Doing this consistently with limited staff is hard.

The challenges

  • Volume. Every discharge is a potential call.
  • Reaching patients. Patients don't answer unknown numbers, and contact details are sometimes wrong.
  • Clinical questions. Calls uncover issues that need a nurse or pharmacist.
  • Follow-up appointments. Patients may not have an appointment scheduled with their primary care clinician.
  • Documentation. Call results must be recorded in the chart.

Opportunity cost of the status quo

Missed follow-up can mean problems go unnoticed until a return visit. Hospitals subject to readmission measures may be affected, and patient experience scores can suffer.

Illustrative arithmetic, not data: if a hospital discharges 400 patients a month and staff spend 10 minutes per call attempt with two attempts on average, that is about 133 hours a month.

Industry solutions

Prioritize by risk. Call patients with higher-risk diagnoses or new medications first.

Use more than one channel. Combine calls with text messages that explain who is calling and why.

Scripts with escalation. Use structured questions and clear rules for when to involve a nurse or pharmacist.

Confirm follow-up appointments. Help patients schedule appointments that weren't booked at discharge.

Document consistently. Record outcomes in the chart in a standard way.
